Gear whine

'88 Paso.
45,000 miles.

In third gear I get a lot of gear whine. I think it is getting worse over time.
What are the options of what it can be?
How serious is it likely to be?
What should I do about it?

Is this at all speeds/rpm??

... or under hard acceleration combined with a power loss, then it could be a slipping clutch.
Is it only in 3rd gear ? In the worst case the teeth of your gear are worn and if pieces brake off, this could lead to an engine damage sooner or later.

Not the clutch-
at all speeds.
the whine follows the rpm with pitch increasing with rpm.
